1095-C Form Request

The Paperwork Burden Reduction Act (PBRA) was signed into law by the United States
Congress on Dec. 23, 2024. The PBRA amends the Affordable Care Act (ACA) and
states that employers are no longer required to provide full-time employees a Form
1095-C Employer-Provided Health Insurance Offer and Coverage statement.

The forms are for your records only and are not included when submitting your federal
tax return. You may request a copy of Form 1095-C from the Finance Department.
Requests must be submitted in writing and may be emailed to ghickey@sabineisd.org
or mailed to the Glenda Hickey, Director of Business Operations at 5424 FM 1252 W,
Gladewater, TX 75647.
